Heartland Health Services - Sheridan is a Medical Center in Peoria, IL. As a non-profit, Federally Qualified Health Center (FQHC), it provides primary care, women’s health, pediatrics, mental health, addiction recovery, specialty services, discount prescriptions, and after-hours care to patients across Central Illinois. The organization serves low-income adults and children, minority populations, individuals without adequate insurance, those facing medical hardship, and people experiencing homelessness. Founded on December 10, 1991, the clinic began with the dedication of volunteers and one paid staff member, with Peoria’s three hospitals supporting the clinic by providing diagnostic testing and inpatient services at no cost. On February 1, 2004, Heartland was designated as an FQHC. The Sheridan location is part of a network that includes sites in Garden, Wisconsin, Olt, Carver, Knoxville, and John H. Gwynn.

The clinical team at Heartland includes physicians, nurse practitioners, physician assistants, and licensed counselors. Sarah Koscica, MD, serves as Interim Chief Medical Officer and Family Medicine Site Medical Director for the Garden site. Other clinicians include John Mihelcic, MD, Director of Behavioral Health and Medication Assisted Therapy, and Brian Bamberger, MD, MPH, FAAP, FACP, who is the After Hours Care Director. The staff also includes certified nurse midwives, psychiatric mental health nurse practitioners, and family nurse practitioners. Heartland is recognized by the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home (PCMH). It is a member of the Illinois Primary Health Care Association (IPHCA) and the National Association of Community Health Centers (NACHC). In 2024, the Health Resources and Services Administration (HRSA) awarded Heartland the Health Center Quality Leader (HCQL) Silver Badge, an honor held by only the top 20% of the 1,500 FQHCs and look-alikes in the country. The organization is also a Gold Level Advocacy Center of Excellence (ACE), a designation awarded by NACHC.

The Sheridan location is situated on Northeast Jefferson Avenue, adjacent to the Peoria County Offices and the Peoria County Regional Office of Education. It is located near the Liberty Bell memorial and Courthouse Square Greenspace, all within a short walking distance of the facility.
