Agape Community Health Center, Inc. is a medical clinic in Jacksonville, FL. As a Federally Qualified Health Center, it provides comprehensive primary care, pharmaceutical services, and behavioral health to both insured and uninsured residents. The organization operates four comprehensive primary care centers, one women's health center, and two 403b pharmacies across the city. Its mission is to ensure accessible care from birth through adulthood, regardless of age, race, gender, national origin, or socioeconomic status. A sliding discount program reduces the cost of care for uninsured patients based on income and family size, and no individual or family is turned away for inability to pay. The center serves a diverse population of 18,000 patients annually with a multicultural, multilingual, and multidisciplinary staff.

Established in 2004 as a Florida 501(c)(3) not-for-profit, the center was co-applied for with the Duval County Health Department to serve insured, uninsured, and underinsured residents. On April 1, 2016, it was independently awarded the designation as the only 330(e) FQHC in Jacksonville. During the COVID-19 pandemic, the center maintained two sites open to the entire city for education, testing, and vaccinations. It also operates a Community Partnership School at Ed White High School in an underserved community. The board of directors includes more than 51% members who are users of the medical services. Leadership includes Mia L. Jones as Chief Executive Officer, Allen Okie, MD as Chief Medical Officer, and a team of providers specializing in family medicine, pediatrics, obstetrics, and allergy care.

The downtown location sits at 120 King Street in Jacksonville. It is located about a tenth of a mile from Technical High School and roughly two-tenths of a mile from McCoys Creek. Beaver Street is approximately three-tenths of a mile away, as is Hollybrook Park. Stockton Street and Edison Avenue are both about four-tenths of a mile from the clinic.
